Basic information:
90% of the people have some sort of denture or mandible divergences that require dental correction. Orthodontic treatment is aimed at the functional restoration of denture and mandibles. Aesthetics nowadays means face aesthetics not only dental aesthetics. Namely, we can significantly modify the so-called midface, thus the roundness of profile, lips and smile.
Today we may initiate orthodontic treatment regardless of age even at 50-60 years in case the state of periodontium is adequate. However, it is optimal to take children to dentist at the age of 6-8 for the first check. The mandible and occlusion divergences are overt in this age, and it is highly important that the dentist, even though not initiating intervention, may keep track of the polyphyodontia, state of regulation of teeth and occlusion until the treatment commences.
During adult treatment, no growing potential is present, therefore treatment shall be performed by tooth moving or removing observing facial aesthetics, or by mandible surgery with orthodontic treatment in case of severe mandible divergence.
Term of treatment may be between one to several years depending on age, dental divergence, applied treatment and type of appliance.
First consultation and treatment plan:
At the first meeting:
we gather information on needs of patients,
we provide information on basic reference regarding orthodontic treatment,
we provide information on possible treatments of the particular patient,
we provide information on the applicable appliance.
At the second meeting:
X ray pictures (panoramic and tele),
photos (face and mouth),
imprints are taken to record the model of denture.
Utilising the above mentioned, the orthodontist carries out written treatment plan tailored to the particular patient. The plan includes the anamnesis, diagnose, treatment plan and the variation of these in case there are more than one solution and the detailed quote and declarations.
Dental Regulating Appliances:
There are two distinct types of braces: removable and fixed appliances.
1. Removable braces:
The ordinary removable appliances are widely known as “nocturnal applianceâ€, however, it is a misbelief, since the ideal wearing term of these appliances is daily 18-24 hours. It has two types: one that covers mandible and the other type that coats both mandibles thus both dental arches. They are chiefly used with the so-called mixed-denture, which contains both permanent and temporary teeth. These braces are pre-treatment appliances, since 99% of the treatments require fixed appliance to achieve the perfect result. Due to the modern fixed appliances, the treatment may be set forth by applying directly fixed appliance without using removable ones.
Invisalign:
This removable appliance provides orthodntic treatment in permanent denture. The appliance is a transparent splint that only covers teeth. One splint shall constantly be worn for two weeks for 24 hours a day except for meals. When the two weeks expire, one shall wear the next splint that further moves the teeth and so on.
Fixed Dental Correcting Appliances:
The fixed dental correcting appliances consist of several elements. Brackets are attached to incisors, canine teeth and premolars and tubes are affixed to molars. Teeth, namely brackets and tubes, are connected by a so-called arch that is regularly replaced, since different phases of treatment require distinct arch.
Soft force dental regulation. Damon technique:
It is an externally fixed appliance method that was developed by the American Dwight Damon. Its main feature lies within the fact that fixation of arch in brackets is not carried out by rubber or wire elements as in the ordinary fixed appliances, it is rather operated by self-locking brackets and tubes that are opened and closed like bolt lock. In the ordinary rubber fixation dragging is very significant between arch and docking element, while in the Damon system, due to the self-locking method, friction is not very considerable, thus tooth may be moved by using 400-600 times less force. From dental perspective, Damon system is the best correcting appliance in metal (Damon-MX) and a more aesthetic (Damon-3) design.
Lingual technique and Incognito appliance:
It is an appliance attached to internal part of teeth. Its advantage is that it is not visible at all. Its drawbacks are that it is a classic appliance (affixation of arch to bracket is carried out by elastic or metal fixing element), one more difficulty gets used to it, and is more inconvenient to wear and keep it and one shall learn to speak with it.
The externally and internally fixed appliances may be combined, therefore it is possible to attach an internal appliance to upper arch and an external to lower arch.
Retaining of teeth subsequent to active phase of orthodontic treatment:
Retaining of teeth after active moving phase is essential. In case it is not performed, the initial state may entirely or partly return and the treatment shall be set forth again. Retaining of teeth consists of two phases: patient shall wear removable and transparent splint for two years in order to let teeth ossify in their new location. In the first year, daily 14,12,10 and 8 hours decreased in every three months, and every second, third, fourth etc. night in the second year. Consequently, retaining with removable appliance ends after two years.
Since front teeth are likely to move after several years, so-called fix retainers (thin metal wires) are attached to internal part of four upper incisors or six front teeth and six lower front teeth. It is very easy to get used to them; they are not visible and perfectly hold teeth.
Getting used to and wearing braces, eating:
Generally, patient gets used to all types of appliance within two weeks, however, individual differences may occur. When wearing, one shall observe the adequate cleaning; appliance normally does not cause problem after getting used to it, but sometimes it may cause uneasy feeling and particular teeth or group of teeth may be sensitive according to phases of treatment and applied appliance.
Dental hygiene during orthodontic treatment:
Removable braces shall be removed when eating and the tooth brush after meals provide adequate operation in the cleaned mouth.
When wearing fixed brace, patient shall brush his/her teeth after meals as soon as possible. Even after having a bite of food: it means that the patient shall take the tooth brush and paste with him/her, since tooth brush shall be performed within 30 minutes subsequent to meals.
In case fixed dental correcting treatment, regardless the perfect mouth hygiene of patient, dental hygienic treatment shall be performed to achieve the best results. The ozone treatment, that is also suggested during the treatment,� prevent the possible formation of carious
